General Questions
We provide outpatient mental health and substance use treatment services, including
assessments, individual therapy, family therapy, group therapy, anger management,
domestic violence (perpetrator) group and structured programs such as Intensive
Outpatient (SAIOP/SACOT).
Our office is located at 415 West Main Ave., Gastonia, NC 28052.
We are open Monday through Friday, 8:30 AM–5:00 PM and Friday 8:0 AM–2:00 PM.

Referrals & Intake
Referrals can be submitted directly through our website using our secure referral form.
You may also call our office for assistance at 704-478-6093.
Yes. Referrals may be made by individuals, family members, providers, DSS, courts,
schools, or community agencies.
Supporting documents can be submitted separately via fax (704-973-9287), email
(referral@sunpathllc.com)
Not always. Requested services are for referral purposes only. Final recommendations
will be made based on clinical findings following a comprehensive assessment.
Insurance & Payment
Yes. Please contact our office to verify coverage.
We offer self-pay options and can discuss payment arrangements and additional
options/funding during intake.
Internships, Practicum, & Supervision
Yes. We partner with graduate programs to provide meaningful internship and practicum
placements for counseling and related fields.
Yes. We offer supervision for provisionally licensed professionals, including LCMHC-
Associates and LCAS-Associates.

Records Requests
Records can be requested through our Records Request Form on the website, via fax or
in person. A signed Release of Information will be required before records can be released.
Please allow 7–10 business days for processing. If your request is time-sensitive, we
recommend submitting it as early as possible and contacting our office directly to ensure
timely delivery.
