Records Request & Release of Information
Clients, guardians, or authorized third parties may request records by completing our Records Request Form. Records may include assessments, treatment plans, drug
screenings, or discharge summaries.

To request copies of your records, a signed Release of Information (ROI) form is required. Please download and complete the hard copy of our ROI form, if we do not already have one on file for you. Once completed, you may submit it to us by:

Please note: For your protection, records will not be released without a properly signed ROI.
Please allow 7– 10 business days for records requests to be processed. If your request is urgent, contact our office directly.
Client Rights & Responsibilities
At Sunpath, LLC, clients are partners in their own care.
You have the right to:
- Confidentiality of your records
- Be treated with dignity and respect
- Participate in decisions about your treatment
- Receive information in a way you understand
- File a grievance without fear of retaliation
You are responsible for:
- Attending scheduled appointments on time
- Actively participating in your treatment plan
- Providing accurate and complete information
- Respecting staff, peers, and the treatment environment

Insurance & Payment Information
Sunpath, LLC offers several options to help make services accessible. During the intake process, our team will review your coverage and discuss the best payment arrangements/options for your situation. We are committed to working with each client to ensure cost is not a barrier to receiving care.
Privacy & Confidentiality
All information you provide to Sunpath, LLC is kept private and confidential in accordance with state and federal law, including HIPAA regulations.
Grievance Process
At Sunpath, LLC, we want every client to feel respected, supported, and heard. If you ever have a concern about your care, services, or experience with our agency, you have the right to file a grievance without fear of retaliation.
How to File a Grievance:
- Share your concern with your provider or any staff member.
- If your concern is not resolved, you may submit a written grievance to Sunpath, LLC administration.
- All grievances will be reviewed promptly, and you will receive a written response within 10 business days.
If you need help writing or submitting a grievance, a staff member will be glad to assist you.
